What If My Insurance Isn’t Listed Above?

If you’re insurance is not listed, that means that Mondo Sports Therapy does not have an agreement with your insurance company. In this case, you can pay the out-of-network rate, which is $185 for the initial evaluation and $125 for follow up treatments. You can save the payment information and submit it to your insurance company for reimbursement. It is up to your insurance company on whether or not they reimburse your care for an out of network facility.

Want to Pay Cash and Avoid Insurance?

We are offering an out-of-network rate of $185 for the initial evaluation and $125 for each follow-up treatment session. Patients who do not want to go through insurance for their services can opt to pay the cash rate. However, you cannot apply this cash rate to your deductible if your insurance company is listed above. In short, you pay cash or you go through insurance, but you have to choose your path.

For Medicare patients

Please be advised that Mondo Sports Therapy does not participate with Medicare. What this means to you is, if you wish, we will gladly to treat you at our self-pay rate.  Due to Mondo Sports Therapy not participating with Medicare, we will not be responsible for filing a claim to either Medicare or your secondary insurance. You must also waive your right to submit your own receipts for reimbursement.  It is undecided whether or not we will be accepting Medicare in the future.
